Permission to Skate Forms

Please be advised that as per Ontario Hockey Federation Rules and Regulations Brampton Hockey is not permitted to provide Permission to Skate Forms until the completion of the OHF Championships. In 2013 the first date permissions will be available is Monday, April 15th. Please note that requests for Permission to Skate for players interested in trying out with other LOR centres and who will be Bantam age and older in the 2013 2014 season must first be approved by the respective coach or coaches via Brampton Hockey. Permission to Skate forms are provided for players from teams that are part of the Lake Ontario Region (LOR) that wish to try out for other teams in the LOR. For more information on the Lake Ontario Region please visit the OHF website. Brampton Hockey will accept requests, by email only, to info@bramptonhockey.com, commencing on April 8th. Please include the players name, date of birth, address, team played for in 12 13 and the team or teams and level (AAA, AA or A) that you are requesting a Permission to Skate form for. Forms will not be available until April 15th at the earliest. You will be contacted by email when your form is ready for pick up.
